Your initial appointment will consist of reviewing and explaining your diagnosis and treatment options. If necessary, surgery can be performed the same day as the consultation. However, the medical history must be reviewed and treatment plan will be identified for the surgical appointment. For same day surgery, please have a designated driver available.
Please assist us by providing the following information at the time of your consultation:
- Your surgical referral slip and any x-rays if applicable.
- Please provide a list of all medications.
- If you have medical or dental insurance, please bring the necessary information. This will save time and allow us to submit your insurance claim form to your insurance carrier.
Important Information
All patients under the age of 18 years of age must be accompanied by a parent or guardian for a surgical appointment.
A review of medical history or clinical examination physical examination will be accomplished at the time of surgery for patients who request IV anesthesia or sedation. Please have nothing to eat or drink six hours prior to your surgery. You will also need an adult to drive you home.
Please alert the office if you have a medical condition that may be of concern prior to surgery.
The clinical nurse specialist is always available to answer any questions. Please bring a comprehensive list of all medications and allergies to your initial appointment.
X-Rays
You may request a copy of your x-rays from your dentist or doctor.If time permits the x-rays can be mailed to our office or you may pick them up and bring them to your appointment. If additional films are necessary, they can be taken at our facility. Dr. Brown is participating with most insurances and can submit claims to either the medical or dental carrier depending on specific benefits and coverage identified by your plans.
